Viva Village Forum: Preparing for a Successful Recovery after a Health Crisis
A panel of experts share their perspectives about how to best navigate the transition from a
hospital stay to supportive care before returning home. Learn about: 1) the hospital’s discharge
procedure and next steps; 2) what to know when going to a skilled care environment; and 3)
caregiver resources, respite care and benefits. No charge. Refreshments served.
